Please print and complete the new patient forms from this website and bring them with you to your first appointment along with your insurance card (if you have dental insurance). Also, if you have any dental records or x-rays from your previous dentist, please be sure to bring them as well.

At this initial appointment, the doctor will perform a comprehensive examination and x-rays (if needed) will be taken. The doctor will determine the appropriate treatment for your dental needs.

Please be aware, a broken appointment is a loss for everyone. Kindly inform us, at least 24 hours in advance, if you are unable to keep your appointments.
